All participants are required to complete and submit the Columbia University program waiver and student health record forms. Please download the forms and complete them as thoroughly as possible. You may need to seek assistance from your student's physician to complete the student health record.

Please upload the completed forms on this page no later than 2 weeks prior to arrival at Columbia University.
